Our Legal Duty
Integrative Acupuncture & Health Coaching (IAHC) is required by law to maintain the privacy of your Protected Health Information (PHI), provide you with notice of our legal duties and privacy practices, and notify you following a breach of unsecured PHI if applicable.
How We May Use and Disclose Your Information
We may use and disclose your PHI for purposes of treatment, payment, and healthcare
operations. We may also disclose information when required by federal or Florida law, for public health activities, health oversight activities, judicial proceedings, or other permitted uses under HIPAA.
Your Rights Under HIPAA
You have the right to:
• Inspect and obtain a copy of your medical records
• Request corrections to your records
• Request confidential communications
• Request restrictions on certain uses and disclosures
• Receive an accounting of disclosures
• Obtain a paper copy of this Notice upon request
Complaints
If you believe your privacy rights have been violated, you may file a complaint with IAHC or with the U.S. Department of Health & Human Services. You will not be retaliated against for filing a complaint.
